You may schedule an appointment by calling our office at (503) 649-4124. When scheduling an appointment with us, we are reserving a specific time for you. We always do our best to be punctual and we kindly request that you provide us with the same courtesy.

If an appointment has to be changed, please give us as much notice as possible so that someone else may use your time. We request a notice of at least 24 hours.

Insurance/Dental Benefits

As a courtesy to our patients with insurance, we are happy to submit insurance claims for you. Please bring your insurance card and benefit booklet (if available) to your first appointment. We will estimate your co-payment as closely as possible by confirming your benefits with your insurance company. Payment/co-payment is due at the time of treatment (you are responsible for the portion not covered by your insurance). Any estimate given is not a guarantee of payment by your insurance. We encourage patients to know and understand their dental plan. All services provided are the responsibility of the patient regardless of insurance.

Fees and Payments

In an effort to keep dental costs down while maintaining an exceptional level of care, we ask that you take care of your fees at the time of treatment.
